You can currently fly with Star Alliance member SAS from Gdansk to Chicago for only €310 (~PLN 1,323). Luggage, however, is not included. Should you want to add 23kg of luggage to your booking, the price would increase by roughly €100.
You have to be fully vaccinated for at least 14 days to enter the United States, with an approved vaccine, unless you are a US resident/citizen. You can read more about the entry requirements at the CDC.

SAS will usually fly you to Copenhagen first, from where an Airbus A350 would take you to Chicago.

The fare rules merely state that you have to stay at least 6 days (or a Sunday) in the USA. There’s availability from August through November and from January until March, though there doesn’t seem to be much availability on Fridays and Saturdays.
